  • Conduct assessments and reviews to identify care needs, specific goals, or desired outcomes to maximise independence
  • Recommend appropriate VHC services and recommend other primary health, allied health or aged and community care services
  • Accurately document all communication in DVA approved program

To help you achieve this, ideally, you’ll have:

  • Experience in the aged or community care sector

  • Experience in customer service, including a professional and courteous manner, and a collaborative ‘can do’ approach (desirable)
  • Experience in assessment, care planning and service coordination (desirable)
  • Knowledge of the veteran community and the impact of military training/experiences on the client’s physical and mental health (desirable)
  • Proficient, clear, and effective written and verbal English communication skills
  • Use technology confidently, including Microsoft Office programs

As a valued employee your experience matters. Look forward to:

  • Being part of a trusted team within a supportive community
  • Not-for-profit salary packaging up to $18,550 per year - offering you more take-home pay with less tax!
