The Role:

Benetas Community Health and Care provides diverse services to older clients within their homes across Victoria. These services include Support at Home, Veterans Home Care, respite services and private in-home care services.

As a Care Coordinator – Case Management, you will support the coordination and delivery of services for older people receiving lower complexity home care packages, including Level 1 and Level 2 packages. Working closely with Care Partners, clinicians and the broader Community Health and Care team, you will help ensure clients receive responsive, coordinated and person-centred support that enables them to continue living independently at home.

In this role, you will:

  • Coordinate service setup, scheduling and referrals in line with client needs, preferences and approved care plans
  • Act as a key point of contact for clients, families, providers and internal teams regarding service coordination matters
  • Support clients to understand their care arrangements, service changes, appointments and available supports
  • Assist with care plan development and reviews, ensuring services align with client goals and assessed needs
  • Monitor client wellbeing and service delivery, escalating changes, risks or increasing complexity to Care Partners or senior clinicians
  • Support monitoring and appropriate use of client budgets and service utilisation in line with program requirements
  • Maintain accurate and up-to-date client records, documentation, service logs and appointment records
  • Respond to service requests and enquiries in a professional, respectful and empathetic manner
  • Contribute to quality, compliance and continuous improvement activities across the CHC team

About You:

  • A genuine passion for supporting older people to live their best lives
  • Strong communication and problem-solving skills
  • Certificate III or Certificate IV in Aged Care, Lifestyle and Leisure, Community Services or equivalent experience
  • Demonstrated administration skills and methods for staying organised
  • The ability to work both independently and collaboratively in a team
  • Intermediate computer skills and ability to navigate systems
  • Respect for diversity and the ability to build trust with clients and their families
