The Role:
Benetas Community Health and Care provides diverse services to older clients within their homes across Victoria. These services include Support at Home, Veterans Home Care, respite services and private in-home care services.
As a Care Coordinator – Case Management, you will support the coordination and delivery of services for older people receiving lower complexity home care packages, including Level 1 and Level 2 packages. Working closely with Care Partners, clinicians and the broader Community Health and Care team, you will help ensure clients receive responsive, coordinated and person-centred support that enables them to continue living independently at home.
